The Steroid Treatment Window and Protocols
The primary objective of medical treatment for SSHL is to quickly reduce inflammation and improve blood flow to your inner ear. Corticosteroids are the primary and most effective treatment.
- Within 48–72 Hours (The Golden Window): This offers the highest chance of recovery. While an estimated 32% to 65% of cases may recover spontaneously, studies show that starting steroid treatment early offers the highest probability of total hearing recovery.
- Within 14 Days: Oral steroids remain standard and effective; however, their efficacy declines the longer you wait.
- 2 to 6 Weeks: The effectiveness of oral steroids drops significantly. Your doctor may suggest direct injections into the ear as a backup option.
- After 6 Weeks: Treatment rarely works at this stage, and the hearing loss is likely permanent.
Note: Although the optimal therapeutic window is 24 to 48 hours, patients presenting at day 5 or day 10 should still seek urgent medical evaluation. Significant hearing recovery is still highly possible within this timeframe if treatment is started immediately.
Oral Steroids
The standard first treatment is a high-dose round of oral steroid pills, such as Prednisone. A typical prescription involves taking a high dose for seven to 14 days, followed by a gradual tapering over the next week to let your body safely adjust.
Ear Injections (Intratympanic Steroids)
If you cannot take oral steroid pills, which is common for people with severe diabetes, high blood pressure, or specific medical conditions, or if the pills do not work, your doctor may suggest direct injections. An ENT specialist will numb your eardrum and use a tiny needle to inject the steroid directly into the middle ear. This allows the medicine to soak straight into the inner ear without affecting the rest of your body.
What to Expect During a Diagnostic Hearing Test
To confirm you have SSHL, you will need to undergo a comprehensive diagnostic hearing assessment consisting of three primary modalities:
1. Pure-Tone Audiometry (Air and Bone Conduction)
This test defines the severity and nature of the hearing loss. You will listen to a series of tones through headphones (air conduction) to see how quiet a sound can get before you can no longer hear it. You will also wear a small vibrating device behind your ear (bone conduction) to test how well your inner ear hears sound waves passing directly through the bone.
- If the bone conduction scores are significantly better than air conduction scores, you have an air-bone gap, confirming a conductive blockage.
- If both air and bone scores drop uniformly by 30 dB or more across three frequencies, the diagnosis of SSHL is confirmed.
2. Speech and Word Recognition Testing
This form of testing measures clarity rather than just volume. Speech Recognition Threshold (SRT) measures the lowest decibel level at which you can repeat words. Word Recognition Scores (WRS) measure your percentage of clarity when words are played at a comfortable, amplified volume. A low WRS score (e.g., below 50%) indicates severe nerve distortion and requires aggressive treatment.
3. Tympanometry and Acoustic Reflex Testing
A tympanometer changes air pressure in your ear canal to measure how much your eardrum moves. A flat line indicates fluid or a perforation, pointing toward a middle ear issue. A normal, peaking line indicates your eardrum is working perfectly, strongly suggesting the problem is deep inside the inner ear.
